"Reads the Retina Like a Specialist."

An AI-Powered Retinal Screening and Clinical Decision-Support Innovation

1. INTRODUCTION: THE CONTINUATION OF THE OPTSAM VISION

Following the establishment and development of Optsam AI Models & Apps, we are proud to introduce another chapter in our journey of innovation: Optsam Fundus (Clinical Master v5).

Optsam Fundus represents our continued commitment to exploring how Artificial Intelligence can contribute to the advancement of eye healthcare, particularly in retinal screening, early identification of potential eye diseases, and clinical decision support.

Our innovation journey began with a broader vision of using technology to address challenges affecting people with visual impairments. Through Optsam aEye, we introduced an approach focused on supporting blind and low-vision individuals through AI-powered accessibility.

This time, we are extending that vision into the clinical environment.

Optsam Fundus is designed to bring intelligent retinal image analysis closer to healthcare professionals, helping them interpret retinal findings, identify potential abnormalities, and make more informed clinical decisions.

2. WHAT IS OPTSAM FUNDUS (CLINICAL MASTER v5)?

Optsam Fundus (Clinical Master v5) is an AI-powered retinal screening model developed under Optsam AI Models & Apps.

The model is designed to analyse fundus photographs, assess retinal structures, screen for selected retinal conditions, and provide visually explained findings to support healthcare professionals.

Its development incorporates a dataset of 5,262 retinal images and a structured 7-Stage Ophthalmic Gateway intended to guide image verification and subsequent retinal analysis.

The system aims to make advanced retinal image assessment more accessible and understandable, particularly in healthcare environments where access to specialised ophthalmic expertise may be limited.

3. CORE TECHNOLOGICAL CAPABILITIES

Optsam Fundus (Clinical Master v5) introduces several integrated analytical capabilities:

A. Seven-Stage Ophthalmic Gateway

A structured image-assessment process designed to verify fundus image authenticity and suitability before proceeding with retinal analysis.

B. Four-Quadrant Retinal Anatomical Mapping

The model maps retinal anatomy across all four quadrants, supporting a more structured assessment of retinal regions and visible anatomical features.

C. Diabetic Retinopathy Screening (Grades 0–4)

The model screens for Diabetic Retinopathy using a five-grade classification framework, ranging from Grade 0 to Grade 4.

Its reported evaluation results include:

  • 98.0% held-out accuracy.
  • 100.0% sensitivity for referable Diabetic Retinopathy.

These figures represent the reported performance of the model under its evaluation conditions and require further independent clinical validation before being interpreted as evidence of real-world clinical performance.

D. Diabetic Macular Edema Assessment

The model incorporates assessment capabilities for Diabetic Macular Edema (DME), including clinically significant macular edema (CSME), to support the identification of findings that may require further clinical investigation.

E. Optic Disc and Cup-to-Disc Ratio Assessment

Optsam Fundus also evaluates optic disc cupping and the Cup-to-Disc Ratio (CDR), providing additional information relevant to optic nerve assessment and glaucoma-related screening.

F. Visual Clinical Explanations

Beyond producing screening results, the model is designed to visually explain its findings.

This approach seeks to help healthcare professionals understand where potential abnormalities are located, what anatomical structures are involved, and why particular findings may require attention.

4. OUR PURPOSE: BRIDGING THE GAP BETWEEN TECHNOLOGY AND COMMUNITY HEALTHCARE

The establishment of Optsam Fundus is driven by a fundamental healthcare challenge: the gap between the growing demand for eye-care services and the availability of specialised diagnostic expertise.

Many communities require accessible eye-health screening services, while healthcare professionals working at different levels of the health system need appropriate technological support to recognise potential retinal abnormalities and determine when referral is necessary.

Through Optsam Fundus, we envision a future in which AI-assisted retinal screening can help:

  • Support healthcare professionals in retinal image assessment.
  • Improve awareness and early identification of potential retinal diseases.
  • Strengthen diabetic eye screening initiatives.
  • Support referral decisions for patients requiring specialised examination.
  • Make retinal findings easier to interpret through visual explanations.
  • Bridge the gap between primary healthcare services and specialised eye-care facilities.

Our objective is not to replace ophthalmologists, optometrists, or other qualified healthcare professionals. Rather, it is to develop technology that strengthens their capabilities and supports the delivery of quality eye-care services.

5. OUR INNOVATION PHILOSOPHY

At Optsam AI Models & Apps, we believe that the future of eye healthcare should not depend exclusively on geographical access to advanced diagnostic facilities.

Technology should help bring knowledge, screening capabilities, and clinical support closer to the communities that need them.

From Optsam aEye, which focuses on accessibility for blind and low-vision individuals, to Optsam Fundus, which focuses on AI-assisted retinal screening, our innovations reflect one continuing vision: applying Artificial Intelligence to address practical challenges in eye healthcare.

We are building beyond individual applications. We are exploring an ecosystem of AI-powered models and applications that can serve both healthcare professionals and the wider community.

6. FOUNDERS AND INSTITUTIONAL VISION

J.M. Samwel – Founder

The original vision behind Optsam and its continued development into an AI-powered eye-health innovation initiative. His professional background in optometry and interest in accessible eye healthcare provide the foundation for the development of these initiatives.

J.M. Emanueli – Co-Founder

Joining the Optsam innovation journey in 2026, J.M. Emanueli contributes to the continued development and future direction of Optsam AI Models & Apps, representing the next generation of collaboration in technology-driven eye healthcare.

Together, the founders share a commitment to developing practical innovations that connect eye-health knowledge, artificial intelligence, and community healthcare needs.

7. OUR FUTURE DIRECTION

Optsam Fundus (Clinical Master v5) is another step towards our long-term ambition of developing intelligent eye-health technologies.

Our future direction includes continued model development, broader retinal image evaluation, independent clinical validation, and exploration of responsible integration into healthcare workflows.

We also envision opportunities to connect our AI models with digital health applications and other assistive technologies to make eye-care knowledge and screening support more accessible.

We recognise that clinical AI must be developed responsibly, with attention to patient safety, data privacy, clinical validation, and professional oversight.

8. CONCLUSION: ONE VISION, MULTIPLE INNOVATIONS

Optsam Fundus is not simply another AI model. It represents the continuation of an idea that has progressively evolved into a broader technological vision.

We began by asking how technology could help people who cannot see the world around them clearly. Today, we are also exploring how technology can help healthcare professionals see and understand retinal findings more effectively.

From supporting people with visual impairments through Optsam aEye to supporting retinal screening through Optsam Fundus, our direction remains the same: making eye healthcare more accessible through innovation.

We believe that the future of eye healthcare requires collaboration between professionals, communities, and technology developers.

This is our contribution towards that future.
